This Privacy Notice explains how your personal information may be collected and processed in connection with your participation in our Healthcare Professionals (HCP) survey initiative (hereinafter, the “Survey”).

The controller of your personal information is: CMI, 2000 Market Street, 29^th Floor, Philadelphia, PA 10103. We collect your personal data from the following sources: directly from you when you participate in the Survey. We collect and process the following categories of personal data: first name, last name, position, telephone number, email address, ethnicity, gender, and any personal data contained in the responses you provide in connection with the Survey.

Your personal data may be processed in connection with the Survey to understand further information regarding how Health Care Professionals want medical companies to provide them with critical information. Information will only be published and shared in aggregate. We do not sell your personal information or share it with third parties, unless required to carry out the objectives defined above.

Your personal data shall be retained for the duration necessary to achieve the objectives pursued (see above). We treat personal data as confidential information.

Please note that by participating in the Survey, you are consenting to us, and anyone acting with our authority and permission, to process your personal information as described above.
